The primary purpose of this position is to provide care coordination services according to the guidelines established by the Pathways Community HUB model to mitigate barriers to health identified through risk assessment. The secondary purpose is to coordinate a Whole Family Approach to Care Coordination across WAGAP’s Youth& Family services. This position will work with individuals and families to navigate and access WAGAP services, community services, other resources, and to adopt healthy behaviors. The Department Director will be responsible for supervising employees within the Pathways program, the White Salmon Youth Center, Family Resources, and other WAGAP sponsored projects as assigned by the Executive Director. This person will also be required to report to funders and create new relationships within the community. They will provide training and support to both Community Health Workers and Care Coordinators ensuring the same standard of care and equity approach is provided to all participants. The Director will provide direct services to clients, including visiting their homes as needed. They will serve in an advocacy role; assuring clients and their families are linked with the appropriate community resources and services for improving self-sufficiency. A primary function of this position is to identify members of the target population or accept referrals for clients at great risk of having serious health problems or adverse outcomes, conduct a standardized risk assessment for development of an individualized plan, connect clients to appropriate care, remove barriers to obtaining care, and measuring and recording the results across the variety of programs.
This is a full time position 40 hours a week, $30.52.
