Are you passionate about helping individuals and families navigate complex health and social service systems? We are seeking a compassionate, resourceful, and community-minded Care Coordinator to join our team.
What You’ll Do
As a Care Coordinator, you will:
- Identify and engage individuals and families who may be at risk for poor health outcomes.
- Conduct comprehensive risk assessments to understand client needs and strengths.
- Develop individualized action plans that support health, wellness, and self-sufficiency goals.
- Connect clients with healthcare providers, social services, and community-based resources.
- Help clients navigate complex systems and remove barriers to care.
- Serve as a trusted advocate and partner for clients and their families.
- Provide services in clients’ homes and community settings as needed.
- Monitor progress, document outcomes, and measure the impact of services provided.
- Collaborate with healthcare providers, community organizations, and support networks to ensure coordinated care.

This is a part- time position 29 hours a week, $22.15.
Includes: accrued hours and paid holidays.
